Material irregularity defined
An MI is any non-compliance, fraud, theft or breach of fiduciary duty identified during the audit that has caused or is likely to cause material financial loss, harm to a public institution, or harm to the public. The definition deliberately reaches beyond financial misstatement.
The accounting officer's duty to act
